PHaMs is an outreach mental health program. The program assists people who have a severe mental illness and cannot manage their daily activities or live independently in the community. The program supports people to set and achieve recovery goals, overcome social isolation and increase connections to the community.
Who we help
We help all people over the age of 16 whose lives are severely affected by mental illness.
How we help
We engage the whole community so that everyone is able to support people with mental health needs. We provide education and support to families to empower them to respond to the person’s needs. We assist and support clients to manage their daily activities and link them with specialist service providers when needed.
What is the referral process?
There is no referral required.
Clients are assessed for NDIS eligibility after they begin using the service.
